All programs require pre-registration. Members may register online or by calling the wellness line at 518-641-4800 or toll free at 1-877-986-2373. Online registrations are confirmed immediately.

The health benefits available to members through their CDPHP contract, which are administered separately, continue to be based on medical necessity and scientifically proven standards of care.
