CDPHP Medicare Choices Group Plans are available to eligible retirees and employees with Medicare Parts A & B living in Albany, Broome, Chenango, Columbia, Delaware, Dutchess, Essex, Fulton, Greene, Hamilton, Herkimer, Madison, Montgomery, Oneida, Orange, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Tioga, Ulster, Warren, and Washington counties.

Q. Can I get extra medicine if I am going to be on vacation?

A. Yes. Medicare plan members should call 1-866-289-2319. All TTY/TDD users may call 1-866-236-1069.

Q. How do I use the eyeglasses allowance?

A. The eyewear allowance varies by plan. Please refer to your Evidence of Coverage or Benefits Summary for your plan's allowance amount.
