Members, and their health, have always been at the heart of CDPHP. As a not-for-profit, community-based organization, CDPHP is in an ideal position to form valuable collaborations with its physicians, hospitals, and provider organizations to improve access to care, foster good health, manage illness, and encourage continuity of care.
