The Rewards for Doing Business
A partnership with CDPHP can be profitable, and especially rewarding. BRAVO, Broker Rewards and Value Opportunities, recognizes and rewards broker office administrative personnel and account managers for a job well done. You can earn points and redeem them for valuable prizes.
BRAVO Materials
If you haven’t already received a BRAVO ID card and a supply of BRAVO stickers, simply complete the BRAVO program request form. CDPHP will verify your eligibility and send you a BRAVO ID card package.
Your BRAVO ID card has a unique number; you’ll need it to register at the BRAVO Web site. During registration, select your own password for the Website. Once approved, you’ll receive a confirmation e-mail.
Points for Plans
BRAVO points are awarded for select sales support activities. You earn points when complete and accurate paperwork is submitted to CDPHP for the following CDPHP Universal Benefits, Inc.® products: PPO, High Deductible PPO, EPO, and POS. (New and renewal HMO paperwork does not qualify for BRAVO points.)

In most cases, accurate paperwork will earn you more than one point. For example, if you send paperwork for a new group on the 14th, you can earn a total of three points; two because it’s a new group, and another because it was submitted by the 15th.
To ensure that your BRAVO points are credited to your account, please use the BRAVO stickers. Print your unique four-digit BRAVO ID number (which appears on the BRAVO ID card) on a sticker and place it on the top of your paperwork. If you misplace or run out of stickers, please call the CDPHP Marketing Department at (518) 641-5000 to request more.
CDPHP staff will credit your BRAVO account and you’ll be able to track your points by logging into your personal BRAVO page. Be sure to check back on the BRAVO site regularly to track your performance. Periodic e-mails will also be sent with reminders and updates on the program.

Electronics, Games and More
BRAVO rewards include electronics, games, and home products such as CD players, satellite radio receivers and digital cameras. Check out the full BRAVO Online Catalog after you log into the BRAVO Web site.
Claiming Your Reward
It’s easy to earn rewards, and just as easy to claim them. Simply log into the BRAVO Web site to review the available prizes on the BRAVO Online Catalog. Allow at least one week for your BRAVO points to be registered. After accumulating sufficient points for the award you’d like, add the item to your cart and follow the prompts. The free merchandise you select will arrive by mail in two to four weeks.
