Simple to Use
Just click the link below to download the necessary forms, then complete and return them to CDPHP to register. A user ID and PIN will be provided to you. (If you’re already a user of CDPHP i*Connect, your existing ID and PIN will be valid for Online Health Administrator.)
Easy Client Access
To help your clients gain access to Online Health™ Administrator, simply download and complete the necessary forms. They will need to fill out one Create New Entity form for each location and separate User Access Request forms for each individual who needs access. The Delegation Request form should be signed by both the employer and the broker if you will be performing Online Health Administrator functions on behalf of a client.
