Easy to Use
Just download and complete the Broker Access Request form, and return it to CDPHP to register. A user ID and PIN will be provided to you in two separate mailings. You need both of these mailings to complete the registration process upon your first login. View the Broker Secure Site Registration Tip Sheet.
To help your clients gain access to the secure employer site, simply download the Employer Secure Access Request form.
