  • High Deductible HMO – Quality meets affordability; low rates and a smart deductible (Gold), and HSA-qualified plans (Silver).

Vision Coverage

Options for vision coverage are available with most plans. Benefits include a combination of eye check-ups and coverage for eyeglasses or contact lenses. Most CDPHP commercial business and individual plan members are eligible for a reimbursement of up to $750 for Lasik eye surgery

Dental Coverage

CDPHP partners with Delta Dental, which covers 80 million people in the U.S. We offer a range of plans for all needs and budgets, including plans that cover everything from preventive care to orthodontic services.

Pediatric Dental Coverage

Pediatric dental coverage is required for small group and individual plans under the ACA. Delta Dental plans through CDPHP include the essential pediatric coverage. If Delta Dental isn’t in a group’s benefits solution this year, CDPHP will ensure their employees and families still receive this required benefit if they don’t have coverage elsewhere.

Healthy Direction

Healthy Direction programs help you save on costs while motivating employees to take control of their health. By completing healthy steps like visiting their primary care physician or getting a biometric screening, employees can earn incentives provided by you such as paid time off and extra HSA and HRA funds.
