If you have a problem or complaint about CDPHP Medicare Choices or the service that we provide, please contact us so that we may attempt to resolve it. Please contact CDPHP Member Services to speak with a representative.
If you've contacted us but we've been unable to resolve your problem or complaint, you can submit a formal complaint with the Centers for Medicare & Medicaid Services (Original Medicare) by completing the CMS electronic complaint form. You do not have to contact CDPHP before you submit a complaint to CMS.
