CDPHP does not share member personally identifiable health or financial information for any purpose other than for the administration, servicing or underwriting of a contract, claim or account, or other purposes permitted by law without a member’s prior written authorization. CDPHP does not share, sell, or rent any personally identifiable information about current or former members.
The Information CDPHP Collects
You should be aware that when you visit CDPHP’s website, it collects certain information that does not identify you personally, but provides it with “usage data,” such as the number of website visitors or the pages visited most often. This usage data helps CDPHP to analyze and improve the functionality of its website.

To send a secure and confidential e-mail communication to CDPHP, you must login to the Secure Member Site. Please note that any other e-mail functionality on the CDPHP.com outside of the Secure Member Site does not provide a secure and confidential means of communication.
