The CDPHP Medicare Choices plans that offer Part D prescription drug coverage (CDPHP Medicare Choices Drug Plans) use a formulary. A formulary is a list of covered drugs selected to meet patients' needs.


CDPHP Medicare Choices Drug Plans Formulary


A formulary is reviewed and updated on a regular basis by medical and pharmacy professionals. If the formulary changes, affected enrollees will be notified before the change takes effect.


Drugs on our formulary are organized into different drug tiers, or groups of different drug types. Your copayment or coinsurance depends on which drug tier your drugs are in. It is important to note that generics are frequently the most cost effective choice.


If you currently take a drug that is not on our formulary, you may appeal to ask us to cover that drug. Refer to your Evidence of Coverage for full details on the appeal process.



Transition Policy

New members joining our plan may be taking drugs that are not on our formulary, or that are subject to certain restrictions, such as prior authorization or step therapy. Please review the Transition Program or the Evidence of Coverage for details.
