HIV / AIDS Case Management
Professional Assistance
The CDPHP case management department is staffed by health care professionals who help members with their HIV care. A team of licensed master social workers and registered nurses work together with to create an individualized HIV plan of care.
With just a phone call to the CDPHP resource coordination department toll-free at 1-800-274-2332, CDPHP members can receive assistance with coordination of their medical care and information about community resources to help them cope with their HIV diagnosis.

- AIDS Drug Assistance Program (ADAP)
- Disability benefits and resources
- Medicare/Medicaid programs
- VA benefits and services
- Pharmacy assistance programs
Medical
- HIV confidentiality — Release of Information form
- Medication adherence
- Prompt follow-up care with primary and specialty care physicians
- Educational needs related to treatment regimen and care
- Nutrition
- Mental health status and substance abuse
- Home care needs (medical and non-medical)

- Referrals to legal services—power of attorney, health care planning
- Ryan White Care Centers
- AIDS Council of Northeast New York
