Insuring Children Up to Age 19

Child Health Plus is a New York State program that provides health coverage to uninsured children up to the age of 19, available throughout the CDPHP service area with the exception of Hamilton County. Premiums are on a sliding fee schedule according to family income.

• Child Health Plus application

Applications and the first month’s premium are due by the 20th of the month for coverage to begin the 1st of the following month. Applications will be accepted in the CDPHP lobby Monday through Friday, 8:30 am to 4:30 pm.
