A point-of-service (POS) plan from CDPHP Universal Benefits, Inc. (CDPHP UBI) provides all of the benefits and services of an HMO, but with the added flexibility of seeing providers outside of the CDPHP UBI network.
You select a primary care physician (PCP) from a large list of providers for easy, affordable access to quality medical care. Your PCP can refer you to participating specialists as needed.

- No charge for well-child care and many other routine preventive health care services, including annual physicals, Pap smears, mammograms, and prostate cancer screening provided in-network.

Please Note: The POS Plan is underwritten by CDPHP Universal Benefits, Inc. (CDPHP UBI). You must comply with CDPHP UBI's managed benefits program as set forth in the contract to receive the maximum benefits for out-of-network services. Failure to do so will result in you being responsible for an additional payment of 50% of the allowed amount up to a maximum of $500 for each service payable, in addition to the applicable copayment, deductible and/or coinsurance. There is a $1,000,000 lifetime maximum on out-of-network care.
