An affordable new range of dental options is now available for groups of all sizes, and may be purchased on a "stand-alone" basis, separate from your medical plan.
Different Types for Different Needs
- Contributory PPO Dental Plan — Employer pays all or part of the premium. The preferred provider organization (PPO) design means you have different coverage levels for in-network and out-of-network care.
- Voluntary PPO Dental Plan — Employee decides whether or not to join the group plan and pays the entire or majority of the premium amount. As with the contributory plan, employee can choose between in- or out- of-network care.
- Indemnity Dental Plan — A lower-cost option that delivers a basic level of benefits. Employer pays all or part of the premium and members may use any dental provider
Within each basic plan type, employers may also choose:
- Deductible level, if any.
- Copayment or coinsurance levels.
- Annual benefit limitations.
A company's plan can be limited to simply preventive services or it can be extended to include coverage for restorative services, oral surgery and even orthodontics.
