Family Health Plus
Insuring Adults Ages 19 to 64
Family Health Plus is a New York State program that provides health insurance coverage for uninsured adults ages 19 through 64 who do not qualify for Medicaid.
Participants must meet specific household income levels to qualify.
Family Health Plus is available in Albany, Broome, Columbia, Fulton, Greene, Montgomery, Rensselaer, Saratoga, Schenectady, Schoharie, Tioga, and Washington counties.

How to Apply
Before you can apply, you must meet with an enrollment facilitator or with staff from your local social services district. Your application cannot be processed without a personal interview.
