Maximum Flexibility and Freedom
A preferred provider organization (PPO) plan offered by CDPHP Universal Benefits, Inc.(CDPHP UBI) gives members the freedom to see CDPHP UBI providers without a referral and visit non-participating providers if they choose.
Our Strong Network is Key
- Members of the PPO can see a doctor within the national network and pay lower copayments or coinsurance. When they go outside the network they pay a higher coinsurance after meeting a deductible.
- No primary care physician or referrals required.
- No charge for well-child care and many other routine preventive health care services, including annual physicals, Pap smears, mammograms, and prostate cancer screening provided in-network.
- Worldwide coverage for emergency care.
