Centers for Medicare and Medicaid (CMS) is the designated enumerator for all National Provider Identification (NPI) numbers and has contracted with Fox Systems to administer the enumeration of all health care providers.
Go to the NPI Enumeration web site to apply online or print out a paper application. You may also call the enumerator for a paper application at 1-800-465-3203.
To apply for an NPI, you will need to have the following information:
Individual Practitioners
(Physicians, Adjuncts, Mid-levels)
- Practitioner Name and Degree
- Social Security Number or IRS Individual Taxpayer ID
- Practitioner Date of Birth
- Country of Birth
- State of Birth (if Country of Birth is U.S.)
- Mailing Address
- Practice Location (No PO Boxes)
- Other Provider ID numbers (UPIN, Medicare, Medicaid)
- Taxonomy Code
- State License Number
