What's New

CDPHP Referral Redesign

  • Effective July 1, CDPHP will no longer require the use of referral numbers. As the role of the primary care physician (PCP) and the concept of the medical home continue to be central to the way we do business, coordination of care will still be expected.

BMI Awareness

  • CDPHP is reaching out to its members regarding the significance of knowing their Body Mass Index (BMI) and how this number affects their health, especially as it relates to childhood obesity. Please help us spread this message and take the time to address this important topic as you care for your patients. Learn more.
