Helping Patients Manage Chronic Conditions

Our disease management programs are designed to improve the health of persons with specific chronic conditions and to reduce avoidable complications, such as emergency room visits and hospitalizations. Here is just a sample of the disease management programs offered to support the care you provide to your patients:



Health Coach Connection

This comprehensive disease management program uses evidence-based information to help members understand their diagnoses, available treatment options, and the potential benefits and risks associated with those options. Specially trained health care professionals, such as nurses, dietitians, and respiratory therapists, are available to support members through telephonic interactions, 24 hours a day, seven days a week. The program is intended to transfer self-care skills to our members and help them work effectively with you, their physician.


Smart Registry

The SMART Registry is a list of your patients and contains pertinent health care information. The registry is mailed directly to you once a year. It is intended to give you a comprehensive overview of your patients with chronic conditions, and to help you identify any gaps in care.


CDPHP Health PartnerSM Program for Asthma

The Health Partner program is specifically for your patients who are challenged or overwhelmed by asthma. Registered nurses are available to help answer questions, educate, and provide support tools to any CDPHP member with an asthma diagnosis. The goals of the program are:


  • Member empowerment through self-management and disease control.
  • Improved quality of life through better breathing.
  • Daily compliance with long-acting asthma control medications.
  • Reduced ER and inpatient rates related to asthma diagnoses.

Video Library

CDPHP has a library of DVDs that address a range of health topics. These videos are available at no charge to our members, and we would be happy to provide you a copy, too. The videos provide evidence-based, unbiased information on treatment options and condition management, and are designed to support an informed dialog between individuals and their physicians. All videos are regularly reviewed and updated to ensure the most current and accurate information. They also come with booklets that provide additional information on each topic.


Health Practice Guidelines

We provide our physicians with access to up-to-date, best-practice, evidence-based medicine guidelines. Access to this information, combined with health coaching provided through Health Coach Connection, can help physicians work with their patients to improve care and outcomes.


Flu Vaccine - General Information

CDPHP recommends that members discuss their need for the flu and/or H1N1 vaccine with their provider. General information:


  • No copayment for the flu or H1N1 vaccine.
  • No copayment for a visit for the purpose of receiving the flu and/or H1N1 vaccine.
  • Prior authorization is not required.
  • CDPHP will reimburse both participating and non-participating providers at its participating provider fee schedule for the administration of the H1N1 vaccine.
  • Providers should use one of the following codes:
    G9141 - Influenza A (H1N1) immunization administration, or
    90470 - H1N1 immunization administration.
