Thank you for selecting CDPHP. It may take up to two weeks for CDPHP to receive your enrollment information. As soon as it's received, we will send you your first bill if a payment is due. If you haven't heard from us after two weeks from when you submitted your enrollment, you may contact CDPHP to check the status.


Do You Have a Payment Due?

Your coverage with CDPHP is not effective until you have made a payment.

 

If you provided your email address during the enrollment process on the NY State of Health marketplace, CDPHP will send you an email with your payment information, as well as a link to make an online payment. You may also print a copy of your bill and mail your payment to CDPHP.

 

If you did not provide an email address during the enrollment process on the NY State of Health marketplace, CDPHP will send your payment information, as well as a copy of your bill, to your home address. You can then mail your payment to CDPHP.

 

What Happens Next

Now that you have enrolled as a CDPHP member through the NY State of Health marketplace, you can expect the following:

  • Your CDPHP member ID card will be mailed to you.

  • A new member kit, including your welcome letter with coverage effective date and your subscriber contract, will be mailed to you.
