2021: A Year of Perseverance and Progress

In 2021, the COVID-19 pandemic again took center stage with the ascendance of the Delta and Omicron variants. In response, the CDPHP® team continued working round-the-clock to safeguard its members, providers, employer groups, and the community at large. CDPHP was well-equipped to meet this challenge, employing innovative strategies to remove barriers to care, all the while continuing to advance our vision for an integrated care delivery system.

  • Quality of care remained a priority during this time, as evidenced by our Medicare HMO and PPO plans, as well as our commercial HMO plans, earning a 5-star top rating from the Centers for Medicare and Medicaid Services (CMS).

  • Our Hospital to Home staff continued – both virtually and at bedside – to help coordinate care and assist with discharge planning for our hospitalized members, allaying their concerns and easing their transition to home.

  • Telehealth solutions, including aptihealth, Doctor On Demand, and Valera, ensured that our members could address physical and mental health issues and stay current with preventive care measures, even when they were unable or reluctant to seek in-person care.

  • The CDPHP care team provided outreach to members overdue for cancer screenings, and we helped support community-based vaccine clinics and provided supplies to combat the spread of Covid-19 infection.

  • Innovative pharmacy programs like MedCheck, ConnectRx Hometown, and our 4-star medication therapy management program ensured that our members could access needed medications, use them safely, and receive medication counseling.

  • And, as always, we continued to develop and expand support to our physician partners, building alliances to enhance practice management and using state-of-the-art data and analytics to improve the delivery of quality primary and specialty care.

Despite the many challenges posed by the persistence of the COVID-19 virus, CDPHP remained strong and steadfast in its commitment to develop an integrated delivery system in the Capital Region and beyond. In 2021, we cemented that vision by announcing a strategic partnership with Community Care Physicians (CCP) that is designed to further enhance the quality and experience of care for CCP patients and CDPHP members.

As we look to the future, we are confident that CDPHP is strong and well-positioned to support the delivery of high-value, high-quality, personalized, and integrated health care throughout Upstate New York.
