Partnering with Community Care Physicians for Better Member Care
We are collaborating with Community Care Physicians, PLLC (CCP) in this effort, removing access barriers, controlling costs, supporting practitioners, and educating members so they can live healthier lives.
In 2022, CDPHP and CCP broke ground on a state-of-the-art medical arts complex that will be our shared headquarters. The aim is to offer patients convenient access to high-quality, coordinated care and services, thereby improving their experience of care, fostering strong patient-doctor relationships, and facilitating communication between practitioners.

Expanded Mental Health Access
The coronavirus pandemic gave rise to a sharp increase in demand for mental health services that outstripped capacity to meet that demand. This crisis in treatment access prompted CDPHP to provide alternative resources for its members.

The Living Room Project. CDPHP has joined forces with Ellis Medicine to help combat the mental health crisis in the Capital Region community via a groundbreaking crisis diversion center. The program offers adults experiencing a mental health crisis no-cost, walk-in access to licensed and caring staff in a comfortable, homelike setting rather than a noisy, overcrowded, and costly hospital emergency room. Services include assessment, supportive therapy, and referral to community resources.
Pharmacy Innovation
One of the key components of our integrated care delivery system is to ensure that our members have convenient access to pharmacists they trust and the medicines they need at the lowest possible cost. CDPHP has taken a multi-pronged approach to this issue, focusing on convenience, collaboration, and cost control.

CDPHP ConnectRx, On the Go. To make it even easier for our members to access needed medications at the lowest cost, CDPHP developed the CDPHP ConnectRx, On the Go app. With the stroke of a key on their mobile device or computer, our members can view specific medications and their cost, search for drug pricing and alternatives, switch pharmacies, receive alerts for screening appointments, and check their eligibility for CDPHP Rx for Less savings.

Medication Management and Pricing. CDPHP offers Medicare members with chronic medical conditions a medication therapy management program to ensure they are taking the right number and kind of drugs at the right dose and least possible cost. All other members can use our MedCheck program to request that a pharmacist review their medications, or CDPHP Price Check to estimate costs according to their individual coverage and benefit package.
Member Education and Screening
In 2022, CDPHP marked the first full year in operation for the 1785 specialty medical practice facility many refer to as the “Doctor’s Office of the Future.” Throughout the year, Albany ENT & Allergy Services, Albany Gastroenterology Consultants, Capital Cardiology, NY Nephrology, and our own ConnectRx pharmacy collaborated to build community awareness of the building and its coordinated care services.
This year, those services included 22 educational and preventive screening events that reached nearly 600 people virtually and more than 400 people in person. Among the topics presented were living well with diabetes, chronic sinusitis, atrial fibrillation, acid reflux, fall prevention, bladder control, advance care planning, and Medicare Advantage.
Hospital to Home
The CDPHP Hospital to Home (H2H) program, which is operational at Albany Medical Center, Ellis, St. Peter’s, Samaritan, and Saratoga hospitals, offers members concierge-level support during and after hospitalization in collaboration with hospital staff. In 2022, the H2H care team supported 18,000+ eligible members, answering questions, facilitating transitions of care, addressing social determinants of health impacting post-discharge recovery, and identifying and closing care gaps.
Also this year, we doubled the size of our social work team in response to member needs, provided more than 15,000 post-discharge meals to 1,115 members, and hired care transition coordinators to assist with appointment setting, transportation, nutritional support, and other measures to ensure continuity of care and improve health outcomes.
Top Doctors
At CDPHP, the connection we share with our network practitioners is unique. Rooted in the success of our nationally recognized Enhanced Primary Care (EPC) program, our CDPHP Top Doctors report celebrates this relationship, recognizing local primary care physicians and practices that excel in the areas of quality, cost effectiveness, and patient satisfaction. This year, we honored the pediatrician, family practice, and internal medicine practice that scored the highest based on our EPC quality measures.
Also supporting the health of our community is a broad array of gifted health care professionals within our network, including nurses, physician assistants, pharmacists, mental health practitioners, and office staff and administrators. To recognize their contributions, we sought nominations from our members and collaborated again with the Albany Business Review to sponsor the CDPHP Top Doctors People’s Choice Awards.
Enhanced Primary Care
Our Enhanced Primary Care program, which is the CDPHP version of a patient medical home, is a model of care that is focused on disease prevention and care coordination, rewarding physicians for the value of care delivered, not the volume of care. In 2022, we introduced several efficiencies to the program in collaboration with our providers.

Working with corporate analytics, we provided network physicians with supplemental HEDIS (Healthcare Effectiveness and Data Information Set) data.

New initiatives were launched to address total cost of care through pharmaceutical changes, improved transitional care management, and identification of care gaps.

We introduced network physicians to innovative programs designed to improve patient health, including Ovia Health™ and the Pivot smoking cessation program.

To address screening gaps, we conducted personalized outreach to patients and provided physician offices with Cologuard® test kits and RetinaVue cameras for disease detection.

Our IT team optimized the prescription delivery radius map on the ConnectRx website for providers.
Specialty Value-Based Payment Program
CDPHP continues to forge innovative relationships with the specialty provider community, promoting shared responsibility and heightened communication between primary care providers (PCPs) and specialists. The aim is to increase access, improve overall patient care and clinical outcomes, and reduce unnecessary hospitalizations and emergency department visits.
Quality is the foundation of this program, which financially rewards specialists who meet or exceed quality metrics and manage chronic conditions effectively. Initially focused on cardiology and renal specialty practices, this specialty value-based program (SVBP) was extended to behavioral health practices in 2022.
To date, more than 13 independent specialty practices are participating in the SVBP, impacting the care of more than 47,000 members across all product lines. Among the innovative clinical management solutions that have been implemented are same-day/walk-in clinics for patients with cardiac and renal conditions, remote dielectric sensing, virtual cardiac rehabilitation, and evidence-based, data-driven management of chronic kidney disease along with early outpatient and home dialysis. In 2022, a network behavioral health valued-based program with enhanced fee-for-service rates was launched to incentivize providers and improve access to care.
Qure
To ensure that our members with cardiac health issues receive the highest quality care, CDPHP continued to work with Qure HealthcareTM in 2022. The program uses clinical simulations, small group discussions, and individualized feedback to help our network cardiologists stay current with national best practices.
Customer Service Training
CDPHP is committed to helping providers in our network improve the health and satisfaction of their patients and also hire and retain competent, caring, and customer-focused staff. In 2022, our enterprise training team continued to offer no-cost, virtual or in-person customer service training for practice staff and lunch-hour leadership sessions for practice managers. A total of 232 provider office staff attended these trainings, which received excellent reviews.
