Only brokers and agents who have successfully completed the CDPHP Group Medicare certification can sell, renew, and receive commission on CDPHP Group Medicare Advantage products. Brokers who are currently certified to sell the Group Medicare product for this selling year and have groups enrolled in the product will automatically be provided with the information necessary to become re-certified for each new selling year. There is a separate process to certify for individual CDPHP Medicare Advantage products. The requirements are not interchangeable.
Certified brokers interested in obtaining a quote for a CDPHP Group Medicare Advantage plan, or brokers interested in becoming newly certified in the CDPHP Group Medicare product program, should contact their CDPHP Large or Small Group Account teams.
CDPHP Medicare Advantage plans are available to eligible individuals who have both Medicare Parts A and B and reside in one of the following counties in New York state: Albany, Broome, Chenango, Clinton, Columbia, Delaware, Dutchess, Essex, Franklin, Fulton, Greene, Hamilton, Herkimer, Jefferson, Lewis, Madison, Montgomery, Oneida, Orange, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, St. Lawrence, Tioga, Ulster, Warren, or Washington.

CDPHP Individual Medicare Advantage HMO and PPO plans

Only brokers and agents who have successfully completed the CDPHP Individual Medicare certification can sell CDPHP Individual Medicare Advantage plans. CDPHP group brokers can use our referral process for Medicare individuals who do not fit a group offering. The Individual Medicare Broker Referral Form must be completed and received by CDPHP before a prospective member contacts us.
CDPHP Medicare Advantage plans are available to eligible individuals who have both Medicare Parts A and B and reside in one of the following counties in New York state: Albany, Allegany, Broome, Chemung, Chenango, Clinton, Columbia, Delaware, Essex, Franklin, Fulton, Greene, Hamilton, Herkimer, Jefferson, Lewis, Madison, Monroe, Montgomery, Oneida, Ontario, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Schuyler, St. Lawrence, Steuben, Tioga, Warren, Washington, or Yates.
