Who can I contact to learn more about my health plan?
For additional details on your health plan, log in to your member account and click on My Member Materials. You can also contact CDPHP member services at (518) 641-3140 or 1-877-269-2134, Monday through Friday, 8 a.m. to 8 p.m.


How do I use my health plan?
When you’re ready to use your health plan at a doctor’s office, please have your CDPHP member ID card with you. With your member ID card, you can activate your online member account. The online account gives you access to the mobile app as well. You are strongly encouraged to download the mobile app to your smartphone to use as your ID card when you are at a doctor's office or pharmacy. With the mobile app, you can also find providers (Find-A-Doc) and track your care.


I don’t have my CDPHP ID card yet. Can I still receive care?
Yes! If you have not received your ID card and are in need of medical care, please call CDPHP member services at 1-877-269-2134, Monday through Friday, 8 a.m. to 8 p.m.


Misplaced ID card?
No problem! Log in to your member account to view and print your ID card and to order a new one. You can also download the My CDPHP Mobile app on your smartphone to have your ID card handy at all times.


How do I search for participating providers by my school and nationally?
CDPHP has more than 825,000 providers in our national network, so you’re covered nationwide, even when you’re not in the area. To find a full list of providers and practices in your area, visit Find-A-Doc online, or download the My CDPHP Mobile app. Under Select Plan Type, choose National EPO/PPO/HDEPO/HDPPO to find the providers and practices in our network.


I take medications regularly. What assistance is available? Is there a way I can access these medications without leaving campus each time?  
There are several options for purchasing, receiving, and saving money on your medications:
You can sign up for the prescription mail services from CDPHP to have your prescriptions delivered directly to you. Learn more.

Save money on your prescriptions, too! Rx for Less is a program for all CDPHP members, offering deep discounts on a specific list of drugs when purchased at Hannaford, Price Chopper/Market 32, ShopRite, Walmart, and more. Many generic prescription drugs are available for as little as a penny a pill. There’s no sign-up process or registration fee. Simply present your CDPHP ID card when purchasing your prescription. How can I get help with mental health and substance abuse?
College can be stressful. If you have mental health concerns, call our 24-hour crisis hotline at 1-888-320-9584, to speak with a live, licensed mental health professional who can connect you to the right resources. You can also visit our mental health support page to learn more.


What about dental coverage?
If you are under the age of 19, you are eligible to use Delta Dental for dental coverage. Delta Dental PPO Pediatric Basic Plan provides benefits through a wide network of dentists across the nation. To find a Delta Dental provider near you, search the Delta Dental dentist directory.


I recently paid out-of-pocket for a service that was covered. How do I submit a claim?
Simply download and complete a claim form. You can mail the completed documentation to the address provided on the form.
