Caremark Customer Care Part D Prescription Questions

Current CDPHP Medicare Advantage members with questions about their Part D coverage should contact Caremark Customer Care:

The phone lines are open 24 hours a day, seven days a week. All TTY/TDD users should call 711.


CDPHP refers to both the Capital District Physicians’ Health Plan, Inc., a Medicare-approved HMO plan, and CDPHP Universal Benefits®, Inc., a Medicare-approved PPO plan. CDPHP is a health plan with a Medicare contract. Enrollment in CDPHP Medicare Advantage depends on contract renewal. To join, you must have Medicare Parts A and B and live in the service area.

* Our hours are 8 a.m. - 8 p.m. seven days a week, October 1 – February 14. From February 15 – September 30, Monday – Friday, our hours are 8 a.m. - 8 p.m. A voice messaging service is used weekends, after-hours, and federal holidays. Calls will be returned within one business day.

CDPHP is an HMO with a Medicare contract. Enrollment in CDPHP Medicare Choices depends on contract renewal.
