If you have questions or concerns about your CDPHP Medicare Choices plan, or a problem or complaint about the service we provide, please contact member services to speak with a representative so that we may attempt to resolve it.

If you've called us but we've been unable to settle your problem or complaint, you can submit a formal complaint with the Centers for Medicare & Medicaid Services (Original Medicare) by completing the CMS electronic complaint form. You do not have to contact CDPHP before you submit a complaint to CMS.

Register for a Senior Health Class

Register online for free wellness classes designed specifically for seniors.

* Our hours are 8 a.m. - 8 p.m. seven days a week, October 1 – February 14. From February 15 – September 30, Monday – Friday, our hours are 8 a.m. - 8 p.m. A voice messaging service is used weekends, after-hours, and federal holidays. Calls will be returned within one business day.

CDPHP is an HMO with a Medicare contract. Enrollment in CDPHP Medicare Choices depends on contract renewal.
