Q. Can I get extra medicine if I am going to be on vacation?
A. Yes. Medicare plan members should call 1-866-289-2319. All TTY/TDD users may call 711.
Q. How do I use the eyeglasses allowance?
The eyewear allowance varies by plan. Please refer to your Evidence of Coverage or Benefits Summary for your plan's allowance amount. If your plan includes an eye glass allowance, please complete a Member Claim Form and submit it, along with an itemized bill, to the address on the form. To access the form, click the Forms & Documents link at the bottom of the page and enter “claim form” in the Search box.
