HIV and AIDS Case Management
The case management department at CDPHP is available to help members deal with their chronic health care needs, including HIV. Whether a member has a new HIV diagnosis, or is currently linked to HIV providers, the case management staff of licensed social workers and registered nurses is ready to work with members, providers, and community agencies to help create an individual HIV plan of care.
Members can call the CDPHP case management department to receive assistance with both the coordination of medical care and for information about community agencies and resources. Case management can assist with financial, medical, and community resources.

Members diagnosed with HIV should begin seeing their infectious disease provider twice annually, six months apart, if not already doing so.
