What is a high-risk pregnancy?
To be considered “high-risk,” a pregnancy is generally one in which the mother, baby, or both are at higher-than-average risk of medical complications due to the mother’s age or medical condition, or because of multiple gestation

How do I take advantage of this program?
You or your doctor can call 1-888-94 CDPHP (942-3747) at any point during the pregnancy to request support through this program.

What can the program do for me?
A case manager will contact you by phone and ask about your current health and concerns. We will work closely with you and your physician to develop a care plan tailored specifically to your needs. The goal is to reduce complications and assure a healthy outcome for both mother and baby. We also have a social work case manager to help you cope with any social and financial needs.
