Review your medications one-on-one with a Care Team pharmacist

MedCheck helps members with prescription drug coverage make the most of their benefit. By talking with a pharmacist at CDPHP, members can get help keeping track of their medications and reducing their risk of a medication-related illness or injury.

It’s important to keep track of your prescriptions and know exactly which medications you are taking and when you need to take them, but sometimes it’s hard or overwhelming. MedCheck is a free program that can help make sure you are using your medications correctly and safely.

MedCheck is available at no additional cost and may be right for you if:

  • You have multiple ongoing conditions like diabetes, arthritis, or COPD;

  • You have trouble affording your medications;

  • You sometimes need help keeping track of when you need to take your medications;

  • Your medications have changed or you’ve been in the hospital recently and now have new or different prescriptions;

  • You are enrolled in a CDPHP Medicare plan with Part D prescription drug coverage but do not qualify for standard medication therapy management services;

  • You are experiencing side effects or have questions about how to best take your medication.

MedCheck is staffed by licensed pharmacists who want to make sure your prescriptions are working well and are affordable for you.

How it works:

  • A pharmacist from the CDPHP Care Team will review your medications one-on-one with you.

  • Then, the pharmacist may give you helpful suggestions for maintaining or improving your health or give you advice to better follow your doctor’s plan.

  • The pharmacist may also talk to your doctor about your medications to see if you can switch to a cheaper drug or one with fewer side effects.

  • The pharmacist may also recommend other community-based or free CDPHP services that may meet your needs.
