What are parts A, B, C, and D?

Part A covers inpatient hospital stays, care in a skilled nursing facility, hospice care, and some home health care. Part B covers certain doctors' services, outpatient care, medical supplies, and preventive services. Part C is a type of Medicare health plan offered by a private company that contracts with Medicare. Medicare Advantage plans provide all of your Part A and Part B benefits. Part D adds prescription drug coverage to Original Medicare and some Medicare cost plans and private fee-for-service plans.

Do I still have to pay my Part B premium if I enroll in a Medicare Advantage plan?

Yes, you are still required to pay your Part B premium to Medicare. The cost for Part B is based on your household income and can differ from person to person. Check with Medicare for the amount you will pay for your Part B premium.

When do I apply for Medicare?

The Initial Enrollment Period (IEP) is the first time you can sign up for Medicare. You may join Medicare Parts A, B, C and D during this window of time: The 3 months before your 65th birthday | The month of your birthday | The 3 months after your birthday

A convenient flex card makes using CDPHP Medicare Advantage dental and vision benefits easy. The full annual benefit is loaded on your card the very first day your CDPHP coverage is active. That means you can use it right away!

Activate your card as soon as you receive it! Call 833-647-9633 and follow the prompts. Make sure you have your health plan ID number available when calling.

Where can I use my flex card?

Your card will work at authorized dental and vision provider locations.

If you do not have your flex card available or are unable to use it for an eligible expense, you may submit a claim for reimbursement.

How do I use my flex card?

Swipe your card at your dentist office or your vision provider office to pay for eligible products and services. After you swipe, be sure to select the credit option on the card machine. If you select debit, your purchase will be declined.

Dental and vision spending amounts are issued on an annual basis.
You have until December 31 each year to use all spending amounts. They do not roll over from one year to another.

How do I report my card lost or stolen?

If your card is lost or stolen, please call us at 1-888-248-6522 ( TTY: 711) to report it. We will issue you a new card.

How do I know how much I have available to spend?

The amount depends on your benefit plan. Check your plan documents, visit your CDPHP online member account at member.cdphp.com, or call 1-888-248-6522 ( TTY: 711) to find out your amount.

Can I use my flex card to buy food, pay bills, or pay for dental or vision products or services for other family members?

Your flex card can only be used to pay for eligible dental and vision products and services for yourself.

If my purchase goes over the amount I have available, will my card subtract money from my other benefits?

Your dental and vision benefit dollar amounts are loaded as two separate categories on your flex card. Your dental benefit will only work for dental services and your vision benefit will only work for glasses and contacts. If your dental or vision purchase is more than what you have available, you can pay the rest with another form of payment like a credit card.
