Strengthening Physician Partnerships

CDPHP is strengthening its position as a trusted health partner across the Capital Region and beyond.

CDPHP and Community Care Physicians (CCP) continued to advance our joint mission of improving the health of the community through our innovative strategic partnership, by shining a spotlight on quality care initiatives and timely health topics for our shared members and patients.

Together in 2025, CDPHP and CCP:

  • Supported important provider and patient conversations through provider office signage and videos, take-home materials, blogs, and targeted emails about:

    • Preventive care, including annual physicals, cancer screenings, and vaccinations

    • Childhood vaccinations, including the importance of starting the HPV series at age 9

    • Diabetes prevention and management

    • Fall prevention

    • Bladder control

    • Medication adherence

    • Antibiotic stewardship

    • Emergency Department and Urgent Care avoidance

  • Co-hosted patient education and wellness events like:

    • Once Upon a Wellness Storytime, Healthy Neighborhood event fostering the health and wellness of children and young families

    • Navigating Autism: Tools for Parents webinar geared towards supporting parents raising children with autism spectrum disorder


In 2025, CDPHP strengthened its clinical leadership with the appointment of Martin Stallone, M.D., as Executive Vice President and Chief Healthcare Services Officer.

In 2025, CDPHP strengthened its clinical leadership with the appointment of Martin Stallone, M.D., as Executive Vice President and Chief Healthcare Services Officer, effective December 1, 2025.

Dr. Stallone brings decades of leadership experience across upstate New York health systems and will oversee key areas including provider networks, quality programs, and member care management. His role is central to advancing the CDPHP strategy to better align providers, insurers, and care delivery systems – enhancing access, improving outcomes, and managing costs.

This appointment reflects the CDPHP commitment to physician-led innovation and deeper integration across the healthcare ecosystem.

Top Doctors Program

Now in its ninth year, the CDPHP Top Doctors Report recognized 50 high-performing primary care practices across the region. These practices excelled in:

  • Preventive screenings and immunizations

  • Chronic disease management

  • Behavioral health screening and support

  • Pediatric care quality measures

  • Patient safety and responsible prescribing

This program reinforces the CDPHP belief that high-quality primary care is the foundation of better health outcomes.

Through programs like Bridgeway (highlighted in regional media coverage), CDPHP continues to partner with innovative providers such as Dr. Oldendorf to expand access to behavioral health services and community-based care solutions.
