CDPHP Care Advantage (HMO C-SNP) Provider Education

CDPHP is launching a new Chronic Special Needs Plan (C-SNP) to serve beneficiaries residing in the Capital District (Albany, Rensselaer, Saratoga, and Schenectady counties) who have one or more of the following conditions:

  • Diabetes mellitus

  • Chronic heart failure (CHF)

  • Cardiovascular disorders (CVD), which are limited to:

    • Cardiac arrhythmias

    • Coronary artery disease (CAD)

    • Peripheral vascular disease (PVD)

    • Chronic venous thromboembolic disorder

Special needs plans are a type of Medicare Advantage benefit plan that offer care coordination for individuals with special needs.

The Centers for Medicare and Medicaid Services (CMS) require providers who care for patients in C-SNP plans to complete an annual training.

Please ensure all providers in your group have completed this training, then submit the form located below the presentation. The attestation only needs to be submitted once for your practice, by an authorized representative on behalf of all providers encompassed under your practice’s tax identification number (TIN). By submitting this form, the authorized representative attests that all providers have completed the C-SNP training, and that all those who join the practice in 2024 will complete the training.
