Receive up to $100* when you participate in a qualifying weight management program.

Here’s how to get started:

  1. Participate in a weight management program

    Pay for and participate in a qualified weight management program or at least four sessions with a registered dietitian. Reimbursement is available once per member, per plan year.

    This benefit does not apply to all plans. Please check your plan contract, call member services, or contact your employer’s health benefits administrator to see if this benefit is part of your health plan.

    Qualified programs include:
    • Counseling with a registered dietitian

    • Medical provider-based programs

    • WW Digital and/or Studio

    • Next Steps at Ciccotti Center ($50 reimbursement) 

    • Kids on the Move at Ciccotti Center ($50 reimbursement) 

    • TOPS (Take Off Pounds Sensibly)

    • Nutrisystem

    • Jenny Craig

    • Optavia

    • Noom

    Exclusions
    • Programs not included in above listing

    • Food and/or dietary supplements (other than those purchased directly from a program listed above) are not eligible for reimbursement

  2. Complete the Weight Management Reimbursement form and submit all documentation

    Complete the Weight Management Reimbursement form. Mail the form, documentation of your participation, and proof of payment to:

    Please allow four to six weeks for processing.


* This reimbursement is available once per member, per benefit period. Maximum reimbursement is $75 for Federal plan members and individual Medicare members, as well as some self-funded members. To confirm your reimbursement maximum, log in to your member account at member.cdphp.com, click on the Benefits & Treatment Costs tab, select Medical Benefits, and type “weight management” in the search box; or call the number on your member ID card.
