The support you need, when you need it most.

A hospital stay can sometimes be scary, confusing, and costly. That is why we created CDPHP Hospital to Home, an exclusive benefit to keep CDPHP members and their loved ones well-informed and supported during and after a hospital stay.

As a CDPHP member, you will receive extra support during a hospital admission at participating hospitals. Our Patient Support Team, which includes nurses, doctors, social workers, mental health care professionals, and member benefit specialists, is available to answer questions regarding your medical care, medications, health insurance coverage, and more.

Once you are discharged, your support continues. Our outpatient care managers will help you with:

  • Follow-up doctor’s appointments

  • Medication assistance

  • Transportation needs

  • Ongoing support
