Medicaid and Medicaid HARP

What is Medicaid? 

Medicaid is a state-sponsored health plan for people who have little to no income. CDPHP offers Medicaid coverage to members in the counties identified in the map below.

How to find out if you qualify for Medicaid
To find out if you quality for Medicaid, fill out this form and a CDPHP representative will contact you. You can also call 1-844-237-4773.

Find out if you qualify

You may qualify for Medicaid if:

  • Your income is below 138 percent of the federal poverty level (approximately $16,754 for a household of one in 2018).
  • You are an adult between the ages of 19 and 64.
  • You are pregnant or have an infant.
  • You are a child between the ages of 1 and 18.
  • You are a parent or caretaker.
  • You are 19 or 20 and live with a parent.
  • You participate in the Family Planning Program.
  • You are not eligible for Medicare.

What is Medicaid HARP?

CDPHP offers a new kind of Medicaid plan called a Health and Recovery Plan, or HARP. This plan offers the same services as our Medicaid Managed Care plan, plus additional benefits called Home and Community Based Services (HCBS) for those who are eligible. When enrolling in the CDPHP HARP plan, you can keep the providers you are currently seeing in your Medicaid Managed Care plan. To find out if you qualify for HARP, or to enroll in our HARP plan, please call New York Medicaid Choice at 1-855-789-4277 or visit New York Medicaid Choice will work with your local county Department of Social Services to confirm your eligibility for this product and enroll you in HARP.

Important Updates

The following updates have been made to your CDPHP Medicaid plan:

North Country Expansion
As of July 1, 2018, CDPHP now offers Medicaid Select Plan and Medicaid HARP coverage in Clinton, Essex, Franklin, and Warren counties.

Harm Reduction Services
As of July 1, 2018, Medicaid Select Plan and Medicaid HARP members now have access to harm reduction services through their benefits. These services offer support to members, including overdose prevention and response. CDPHP will cover harm reduction services, including:

  • A plan of care developed by a person experienced in working with substance users
  • Individual supportive counseling
  • Group supportive counseling
  • Counseling to help you with prescribed medication and continuing treatment
  • Support groups to help you understand substance use, and to identify coping techniques and skills

If you are currently receiving harm reduction services from a provider, your care will not change; you can still go to the same provider to receive these services.

Delta Dental
As of July 1, 2018, Medicaid Select Plan and Medicaid HARP members will have a new dental network, managed by Delta Dental. You will no longer be required to list a primary care dentist as part of your coverage, and will be able to go to any dentist within the new network.

Medical dental treatment, such as fracture repair, tumor removal, or treatment of accidental injury, will still be coordinated by CDPHP.

Speak to a Mental Health Professional
If you need to talk to someone about mental health or substance use disorder concerns, call our Behavioral Health Access Center at 1-888-320-9584 Monday through Friday, 8 a.m. to 6 p.m.

Need Help After-Hours?
If you have an emergency and need to speak to a mental health professional after-hours, call CONTACT Lifeline at 1-855-293-0785. You will be immediately transferred to a live, licensed counselor for emotional and crisis support. CONTACT Lifeline is available Monday through Friday, 6 p.m. to 8 a.m., and on weekends and holidays.

Request Prior Authorization
Some health care services need approval by your health plan before you receive them – this is called a prior authorization. To request a prior authorization, you or you doctor can call 1-800-274-2332, or fax a request to (518) 641-3207.

File an Appeal
If you think our decision about your prior authorization request was wrong, you can ask us to look at your case again – this is called a plan appeal. To file a plan appeal, you can call 1-800-388-2994, or fax your plan appeal to (518) 641-3401.
